NEW! OhioRISE
We are pleased to offer Behavioral Health Respite through OhioRISE.

Behavioral Health Respite is a service that provides short-term, temporary relief to the primary
caregiver of an OhioRISE plan enrolled youth, to support and preserve the primary caregiving
relationship.
How do I get started?
2
Initial meeting with Care Coordinator
Ask your Care Coordinator to send us a Release of Information "ROI" form so we can discuss the needs of the household and OhioRISE recipient.
3
Respite Approved!
OhioRISE allows up to 90 days of services without a prior authorization. If more days are needed, an authorization needs to be approved by your Care Coordinator.
4
Behavioral Health Respite Begins
A provider of your choice or an outside hire will begin services according to the youth's Child and Family Care Plan (CFCP). We will be in regular contact with you and the Care Coordinator to ensure needs are met.
